Am I Eligible for SCN Services?
Anyone with Medicaid can get screened for eligibility. Medicaid Managed Care members who have certain health conditions or social needs may qualify for one-time or short-term support services through the Social Care Network, on top of the benefits they already have. More details are available in the FAQs below.
Even if you don’t qualify for this specific program, you may still be connected to similar help through other city, state, or federal programs. A short, free screening can point you to the best options.
How do I know if I’m eligible for Social Care Network services?
You may qualify if:
- You have Medicaid Managed Care
(Learn more about new Medicaid recertification rules and resources) - You have unmet social needs like not having enough healthy food, safe housing, or reliable transportation
- You’re part of an Enhanced Service Population, which may include:
- Children
- Adults
- Members with behavioral health needs
- Pregnant persons
- Other Medicaid members who could benefit from extra support

What if I’m already enrolled in a Health Home program?
You can still get screened for the Social Care Network. The two programs are meant to work together. Your Health Home care manager can help you complete the SCN screening to see if you qualify for extra services—like help with overdue utility bills, mold removal, or medically tailored meals—that Health Home may not cover on its own. Being in both programs will NOT affect your Health Home enrollment or your Medicaid benefits.
Is it free to enroll in the Social Care Network?
Yes. If you have Medicaid and meet the program requirements, it’s completely free to join a Health Home or get screened for the SCN.
Can my whole family participate?
Yes, if anyone in your household has a Medicaid number, they can be screened and get the help they need. Everyone may qualify for different services based on their unique needs.
What if I already received help from a food pantry or housing agency?
You may still qualify for other types of support, such as:
– Extra food assistance
– Medically tailored meals
– Help with overdue utility bills
– Mold removal or home accessibility upgradesWill I lose my benefits if I don’t join?
No. These programs are completely voluntary and won’t impact your Medicaid. You can join or stop at any time.
